Why every company vehicle needs one
After a collision, drivers are shaken and details disappear fast: the other driver's insurer, a witness's phone number, the police report number. A printed form in the glove box tells the driver exactly what to collect, in order, and gives your office and insurer one consistent record. It is an internal report for your company. It does not replace a police report or your insurer's claim form; in Quebec, that is the constat amiable.
How GPS tracking and dash cams add the facts
High Point GPS tracking records the time, location and speed of every trip, and flags harsh braking and collision events, so you know where and when it happened without waiting for a call. Geotab GO Focus Plus and GO Focus Pro AI dash cams record video of the event. Together they give you evidence to set beside the driver's report.

Safety first. Drivers should check for injuries and call emergency services before filling in anything.
Print the form with your company's contact details and keep a copy and a pen in every vehicle.
Take photos of all vehicles, plates, the scene and road conditions, and tick the photos box on the form.
Drivers should state facts only and never admit fault at the scene. The insurer decides responsibility.
Questions about accident reports
- What should a vehicle accident report include?
- It should record the date, time and exact location, weather and road conditions, the driver and company vehicle, the other driver's name, licence, plate, insurer and policy number, witness contacts, the police service and report number, any injuries, a description of what happened and the damage to each vehicle. This form has a field for each of those.
- Does this form replace a police report or insurance claim?
- No. It is an internal record for your company, so the driver collects the same facts every time. Police reports, insurance claims and, in Quebec, the constat amiable are separate documents. Whether police must be called depends on the province or state and the circumstances, so check the rules where your vehicles operate.
- When should the driver fill in the form?
- At the scene, once everyone is safe and emergency services have been called if needed. Details such as the other driver's insurer, plates and witness phone numbers are easy to collect on the spot and hard to recover later. The driver can complete the description and sign once they are back at the office.
- Is anything I type sent to High Point GPS?
- No. The form runs entirely in your browser, and nothing you type is sent to us. Your entries stay in this browser on this device until you clear them. Most fleets fill in the company details here, print blank copies for every vehicle, and keep completed reports with the vehicle's file and the insurance claim.
- How can dash cam video help after a collision?
- A dash cam records what happened in front of the vehicle, and on some models inside the cab, at the moment of the event. That video can confirm or correct what each driver remembers. With GPS data showing the time, location and speed, it gives you and your insurer more than the drivers' accounts alone.
